The tear trough is the groove that runs under the eye between the lower eyelid and the cheeks. See diagram. Prominent tear troughs appear as under eye hollows and dark circles.

What causes under eye hollows and dark circles?

Under eye hollows may be hereditary and can occur at relatively young ages. They may also develop following overdone under eye surgery.

Most commonly they occur as we age. As the fat directly under the eye begins to protrude with age, it can overhang and overshadow the tear trough, making it seem relatively deeper and darker and interrupting the previously smooth transition from the eyes to the cheeks.

Whatever their cause, prominent tear troughs or under eye hollows and dark circles contribute to a face that looks tired, prematurely aged or chronically ill.

Treatment of under eye hollows.

Until recently, treatment of under eye hollows, dark circles and bags involved surgery, in which excess skin and fat were removed via an incision created just beneath the lower eyelash line or inside the lower lid. Not surprisingly, one of the risks of this surgery is that if too much volume is removed the under eye area can end up looking even more hollow, gaunt and aged instead of rejuvenated.

Over the last few years, a less invasive and safer alternative treatment has become increasingly popular. Rather than removing under eye fat, natural injectable dermal fillers* are used to fill and shape the tear trough itself. This “natural replacement therapy” effectively camouflages eye hollows and restores volume to create a smoother, more youthful transition from the eyes to the cheeks. Because dark circles are often the result of shadows thrown on the tear trough by overhanging fat, by lifting and filling the tear trough these shadows are eliminated.

Does it hurt?

Most patients find the procedure tolerably comfortable and require no anaesthesia. However, for those who are sensitive, anaesthetic cream can be applied beforehand and/or laughing gas can be supplied.

How long does it last?

Treatment is very long lasting! Because under eye fillers are placed deeply, they are not subjected to the stresses, strains and movement that cause fillers in other areas of the face to break down more quickly. Therefore results can be expected to last between 12-18 months for most patients.

What are the risks?
